During the course of your employment as an Occupational Therapist with North Devon Healthcare NHS Trust between April 2003 and 10 May 2013:


1. You were unable to manage complex tasks which resulted in your demotion in August 2010


2. In October 2011, you were unresponsive to verbal prompts and enquiries from fellow colleagues


3. In April 2012, in relation to two patients who were discharged from your ward, you:


a. Did not conduct Occupational Therapy assessments


b. Did not complete the ward log case records


4. In April 2012, you inaccurately recorded that a patient had been discharged to A&E


5. In May 2012, a Clinical Support Worker needed to remind you of tasks and appointments on a regular basis


6. On 16 August 2011, whilst transporting a Patient to undertake a home assessment, you:


a. Broke the journey to the patient's home to stop at a local beauty spot and reminisce ;


b. Did not acknowledge the patient's comment that they could not see the view owing to being blind


c. Had to be prompted by your colleague to continue the journey to the patient's home


d. Made the patient repeat a shower stool position assessment approximately five times


e. In assessing mobility issues in the patient's lounge, advised the patient to remove her sofa completely, instead of making minor adjustments to the position of other furniture


7. On 7 March 2013, you were assigned "Patient A" and


a. You did not:


i. Set out SMART goals


ii. Promptly organise the access visit


iii. Attend the access visit in a timely manner


b. produced a report in relation to Patient A which you omitted:


i. Furniture measurements


ii. Equipment needs


iii. Flooring types


iv. Heating recommendation


v. A summary


8. On 8 March 2013, when assessing "Patient B" you


a. Did not record that B's wife stated she could not cope with B at home


b. Did not complete a PADL assessment


c. Were unable to complete the discharge of B


9. The matters set out in paragraphs 1 - 8 constitute lack of competence


10. By reason of your misconduct and/or lack of competence your fitness to practise is impaired
