Order

The Registrar is directed to impose the following conditions on the registration of Mr Collins Esona for the remainder of the period of the Interim Order (namely until 8 December 2019).

1. You must not work in the field of Haematology as a Biomedical Scientist. This will include the areas of Coagulation, Haematology and Transfusion.

2. You must promptly inform the HCPC if you take up employment / Locum work as a Biomedical Scientist.

3. You must promptly inform the HCPC of any disciplinary proceedings taken against you in your role as a Biomedical Scientist by your employer.

4. When employed as a Biomedical Scientist, you must place yourself and remain under the supervision of a workplace supervisor. This supervisor may be off site at a particular time but must be available to give advice and / or assistance (e.g. by phone). This supervisor may delegate this task if he / she is to be unavailable for any period longer than a day.

5. You must supply details of your supervisor to the HCPC within 4 weeks of the operative date when such person becomes your supervisor. You must attend upon that supervisor as required and follow their advice and recommendations.

6. When employed as a Biomedical Scientist you must meet with your supervisor on a monthly basis to review your standard of practise in your role. You must provide a report or reports from your supervisor not later than 28 days and not earlier than 56 days before this Order is reviewed.

7. You must not work in any Locum post or fixed term contract of less than 6 weeks duration.

8. You must inform the following parties that your registration is subject to these conditions:

A. any organisation or person employing or contracting with you to undertake work as a Biomedical Scientist;

B. any agency you are registered with or apply to be registered with as a Biomedical Scientist (at the time of application);

C. any prospective employer seeking to employ you as a Biomedical Scientist ( at the time of application).

9. You will be responsible for meeting any and all costs associated with complying with these conditions.

Notes

This Order will be reviewed by the Committee no later than 6 March 2019 or earlier if new evidence which is relevant to the Order becomes available after it was made.
