The Panel varied the Conditions of Practice Order made on 15 March 2018, and varied on 13 September 2018:
1. You must restrict your practice as a Paramedic to employment with South Central Ambulance Service, NHS Foundation Trust.
2. You must place yourself and remain under the supervision of a workplace supervisor registered by the HCPC (this could be your current Team Leader) and supply details of any change to the identity of your supervisor to the HCPC within 7 days. You must continue to meet with your supervisor on a monthly basis and follow their advice and recommendations.
3. You must promptly inform the HCPC if you cease to be employed by your current employer.
4. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.
5. You must inform South Central Ambulance Service, NHS Foundation Trust, that your registration is subject to these conditions.
Notes
This order will be reviewed by the Committee no later than 5 March 2019, or earlier if new evidence which is relevant to the Order becomes available after it was made.
