Allegation

Whilst registered as a Social Worker and employed by the London Borough of Hillingdon, you:

1. Between 22 July 2016 and 9 August 2016, following receipt of a letter dated 22 July 2016 from Dr X, a Freedom from Torture Counsellor, which related to the concerns and/or risks in respect of Service User A's suicidal ideation, did not:
a) Access previous risk assessments on the Integrated Children’s System recording system (ICS);
b) Undertake a further risk assessment;
c) Update the risk assessment on ICS;
d) Change the risk rating level on ICS;
e) Devise a strategy to deal with the potential risks;
f) Advise your Line Manager and/or Deputy Manager of the potential risks to Service User A;
g) Adequately advise Service User A’s Link Worker and/or the Team of the potential risks to Service User A;
h) Put in place and/or undertake adequate monitoring of Service User A;
i) Maintain accurate records on ICS;
j) Undertake the necessary safeguarding measures.

2. Did not store confidential information in relation to service users in an appropriate manner.

3. In relation to an audit conducted by Colleague A, did not ensure that all relevant risk assessments for service users were:
a) created;
b) reviewed and/or updated regularly;
c) completed to a satisfactory standard.

4. Did not complete a Locality Risk Assessment for Charville Lane Children’s Home.

5. You informed your line manager that you had completed a Locality Risk Assessment for Carville Lane Children’s Home when you knew this was not the case.

6. Did not ensure that one or more of the 12 generic risk assessments required for Charville Lane Children’s Home were completed.

7. Between March 2016 and August 2016 did not ensure that hourly checks on young people within the Service were being completed consistently and/or recorded.

8. Between April 2015 and August 2016 in relation to staff supervision, did not:
a) Conduct regular supervision sessions with staff members you were responsible for supervising directly and/or did not complete satisfactory records of those supervision sessions;
b) Ensure supervision sessions were being conducted regularly for members of staff you were not responsible for supervising directly and/or did not ensure that satisfactory records of those supervision sessions were completed;
c) Update the supervision tracker monthly.

9. Did not ensure that staff working at Charville Lane Children’s Home undertook all mandatory training.

10. Did not ensure that your own mandatory training was up to date.

11. You informed your line manager that your mandatory training was up to date when you knew this was not the case.

12. In relation to a sample of cases looked at by Colleague A, did not ensure that care plans were:
a) created;
b) reviewed and/or updated regularly;
c) completed to a satisfactory standard.

13. In relation to a sample of cases looked at by Colleague A, did not ensure that records relating to missing service users were completed:
a) consistently; and/or
b) adequately.

14. Did not complete and/or update a Statement of Purpose for Charville Lane Children’s Home.

15. You informed your line manager that you had completed and/or updated a Statement of Purpose for Charville Lane Children’s Home when you knew this was not the case.

16. The actions described in particulars 5, 11 and 15 were dishonest.

17. The matters set out at particulars 1 to 16 constitute misconduct and/or lack of competence.

18. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
