Allegation

During the course of your employment as an Orthotist at Royal Wolverhampton NHS Trust, you:

1. Did not record patient notes for approximately 55 patients as shown in in Schedule A, including:
(a) Patient 2 in respect of an appointment held on 3 September 2014;
(b) Patient 4 in respect of an appointment held on 30 September 2014;
(c) Patient 5 in respect of an appointment held on 2 October 2014;
(d) Patient 9 in respect of an appointment held on 25 November 2014;
(e) Patient 17 in respect of an appointment held on 13 November 2014;
(f) Patient 19 in respect of an appointment held on 21 November 2014; and/or
(g) Patient 20 in respect of an appointment held on 25 November 2014.

2. Did not record patient notes accurately and/or in a timely manner for approximately 31 patients, as shown in Schedule A, including:
(a) Patient 1 in respect of an appointment held on 3 September 2014;
(b) Patient 6 in respect of an appointment held on 15 October 2014;
(c) Patient 7 in respect of an appointment held on 15 October 2014;
(d) Patient 8 in respect of an appointment held on 16 October 2014;
(e) Patient 10 in respect of an appointment held on 22 October 2014;
(f) Patient 11 in respect of an appointment held on 22 October 2014;
(g) Patient 12 in respect of an appointment held on 23 October 2014;
(h) Patient 13 in respect of an appointment held on 23 October 2014;
(i) Patient 14 in respect of an appointment held on 23 October 2014; and/or
(j) Patient 16 in respect of an appointment held on 30 October 2014.

3. Did not complete and/or record and/or order patient prescriptions promptly and/or correctly and/or at all for approximately 40 patients as shown in Schedule A, including:
(a) Patient 3;
(b) Patient 4;
(c) Patient 6;
(d) Patient 7;
(e) Patient 8;
(f) Patient 9;
(g) Patient 10;
(h) Patient 11;
(i) Patient 12;
(j) Patient 17;
(k) Patient 18;
(l) Patient 19;
(m) Patient 20;
(n) Patient 24;
(o) Patient 25;
(p) Patient 27; and/or
(q) Patient 28.

4. Did not arrange appointments and/or set a review date on the system for approximately 52 patients as shown in Schedule A, including:
(a) Patient 13;
(b) Patient 14;
(c) Patient 15;
(d) Patient 16;
(e) Patient 18; and/or
(f) Patient 19.

5. In respect of Patient 21:
(a) Did not contact the patient’s mother regarding the need to change the treatment plan in a timely manner;
(b) Did not contact the consultant
(c) Told your manager that you had contacted the consultant when this was not the case.

6. Did not respond to queries from patients’ relatives, colleagues and/or other professionals, in respect of:
(a) Patient 22;
(b) Patient 23;
(c) Patient 24;
(d) Patient 25;
(e) Patient 26;
(f) Patient 27;
(g) Patient 28; and/or
(h) Patient 19.

7. Refused to assess Patient 29 at West Park Hospital on 23 September 2014.

8. In relation to Patient 30:
(a) Did not register the patient on the OPAS system;
(b) Did not record any patient notes in relation to this patient;
(c) Did not place an order for a Cervical Thoracic Lumbar Sacral Orthosis;
i) Following an assessment on 21 November 2014

9. Did not add any ward patients to the clinic list:
(a) on 3 November 2014; and/or
(b) on 21 November 2014.

10. On 23 September 2014, did not give prescriptions for ward patients at West Park Hospital to the physiotherapists.

11. On 26 November 2014, you:
a) threw yourself into filing cabinets;
b) sat rocking in a chair;
c) were visibly distressed in full view of patients and visitors.

12. The matter described in paragraph 5(c) was dishonest

13. The matters set out in paragraphs 1 – 12 constitute misconduct and/or lack of competence.

14. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
