Whilst registered as an Arts Therapist;

1. Between September 2014 and April 2015, you did not ensure that all of

your clinical activity with service users was appropriately recorded, this included for;

a. Service User 1;

b. Service User 2;

c. Service User 3;

d. Service User 4;

e. Service User 5;

f. Service User 6;

g. Service User 7.

2. Between September 2014 and April 2015, did not ensure that service user records were updated in a timely manner, this included for;

a. Service User 1; and/or

b. Service User 4.

3. You treated Service User 4 following your disciplinary process in 2012 with:

a. No clear clinical justification and/or recording of it; and/or

b. No treatment plan and/or recording of it.

4. Between approximately July 2013 and April 2014 you did not arrange any clinical supervision for yourself in respect of your work at the Trust.

5. The matters described in paragraphs 1 - 4 constitute misconduct and/or lack of competence.

6. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
