The Interim Conditions of Practice Order made on 2 February 2018 is varied. The conditions are:

1) You must make arrangements for the testing of your health condition within 30 days of the date of this hearing. You must provide to the HCPC details of the testing arrangements and forward copies of the test results to the HCPC within fourteen days of them being received by you.

2) You must continue to consult your General Practitioner about your health condition and follow any advice and guidance.

3) You must provide to the next panel reviewing this Order a report from your General Practitioner in relation to your health generally and the advice given to you.

4) You must provide to the next panel reviewing this Order detailed evidence of what steps you have taken to address the health concerns identified, including any support you have accessed or attempted to access.

5) You must inform the following parties that your registration is subject to the above conditions:

A) Any organisation or person employing or contracting with you to undertake professional work;

B) Any agency you are registered with or apply to be registered with (at the time of application); and

C) Any prospective employer (at the time of your application).

This Order will be reviewed by the Committee no later than 31 October 2018 or earlier if new evidence which is relevant to the Order becomes available after it was made.
