During the course of your employment as a Paramedic with South Central

Ambulance Service between January 2016 and September 2016 you;

1. On the 24 or 25 March 2016, attended patient 1 and;

a. did not complete a primary survey; and/or

b. did not immediately provide them with oxygen.

2. On the 30 or 31 May 2016,

a. attended patient 2 and;

I. did not undertake a full assessment; and/or

II. did not recognise it was a ‘time critical’ incident.

b. attended patient 3 and;

I. discharged the patient on scene without understanding the nature of their

condition; and/or

II. did not request any assistance in regards to understanding the patient’s


c. attended patient 4 and;

I. Were unaware of the possible consequences of an Amitriptyline

overdose; and/or

II. Were unaware of the signs and/or symptoms of Diazepam overdose.

d. attended patient 5 and did not include sporadic incontinence within your

history taking.

e. attended patient 6 and;

I. did not feel for a radial pulse; and/or

II. did not understand the patient’s ECG interpretation.

3. On the 1 July 2016, attended Patient 7 and did not provide them with


4. On the 2 or 3 July 2016,

a. attended patient 8 and;

I. did not carry out a visual inspection of their airway;

II. did not position the patient correctly; and/or

III. did not consider prescribing pain relief.

IV. were unable to identify a potential differential diagnosis;

V. did not assess Patient 8’s respiratory rate.

b. attended patient 9 and;

I. did not obtain an ECG during the secondary survey;

II. did not consider prescribing pain relief.

III. did not identify an appropriate disposition plan.

c. attended patient 10 and;

I. did not carry out a visual inspection of their airway;

II. did not obtain a respiratory rate;

III. did not consider the patient had back pain; and/or

IV. did not communicate the side effects of the drug administered to the


V. before administering a second dose of Morphine failed to check the

following until prompted to do so:

a) Pulse;

b) Respiratory rate;

c) Blood pressure;

VI. did not identify air in the syringe which you were about to use to

administer Morphine.

d. Attended patient 11 and;

I. did not complete a primary survey; and/or

II. delayed conveying the patient to hospital.

5. On the 3 or 4 July 2016, attended patient 12 and did not complete a

primary survey.

6. Demonstrated poor communication skills with colleagues and/or

patients, which included

a. Communicating your decisions to patients; and/or

b. Delegating tasks to other crew members.

7. The matters as set out in paragraphs 1-6 constitute a lack of


8. By reason of this lack of competence your fitness to practise as a

Paramedic is impaired.
