While registered as a Radiographer and while working as the Imaging Department Lead at
Benenden Hospital Trust, you:

1. On 20 July 2016, in relation to Patient A, did not carry out the Consultant Orthopaedic Surgeon's request for Patient A to undergo a Skull x-ray and changed Patient A’s imaging
request form to a Head CT scan, no contrast, and did so:

a) without consulting the requesting clinician;

b) without seeking agreement and/or authorisation from a Consultant Radiologist;

c) without obtaining and/or documenting Patient A’s consent to proceed;

2. Your actions at paragraphs 1 resulted in Patient A receiving an exposure to an excessive
radiation dose.

3. On 16 June 2016, in relation to Patient B, you;

a) x-rayed Patient B’s chest which resulted in Patient B receiving a dose
of ionizing radiation that was not as low as reasonably practicable
in that you:

b) used a grid contrary to the hospital’s protocol for chest x-ray

c) used a grid focused at 110 centimetres instead of one focused
at 180 centimetres;

d) did not ensure that exposure factors for a high kV (grid)
technique were selected;

e) failed to collimate the images adequately and/or at all.

4. Your actions above resulted in Patient B receiving a radiation dose of
approximately of 85 centigray (cGy) per centimetre of Radiation which was
significantly in excess of the recommended dose;

5. In relation to Patient B, you undertook a patient examination using a piece of
equipment that you were not deemed competent to use, contrary to the Ionising
Radiation (Medical Exposure) Regulations.

6. The matters set out in paragraphs 1 - 5 constitute misconduct and/or lack
of competence.

7. By reason of your misconduct and/or lack of competence your
fitness to practise is impaired.
