The Registrar is directed to annotate the HCPC register to show that for a period of 18 months from the date that this Order takes effect (the Operative Date) you Stephen Mullender-Lock must comply with the following conditions of practice:


1. Except in emergencies [the nature of which you must record in writing and submit that report to your line manager within 24 hours of the event]  you must, when visiting a person for whom you are providing professional services or their families, always be accompanied by another member of staff.


2. You must work with your line supervisor or a person delegated by your line manager who is registered with the HCPC to formulate a Personal Development Plan which will describe your understanding of appropriate professional boundaries and how you intend to ensure that you adhere to them.


3. Within 3 months of the Operative date you must forward a copy of that Development Plan to the HCPC


4. You must meet with your line manager or his/her  nominated delegate, on a monthly basis, to consider your progress to achieving the aims set out in your Personal Development Plan


5. You must allow your line manager or his/her delegate to provide the HCPC with information as to your progress in achieving the aims set out in your Personal Development Plan

6. You must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment.

7. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

8. You must inform the following parties that your registration is subject to these conditions:


 A. any organisation or person employing or contracting with you to undertake professional work;
 B. any agency you are registered with or apply to be registered with (at the time of application); and
 C. any prospective employer (at the time of your application).

Notes

This order will be reviewed by the Committee no later than 10 April 2019 or earlier if new evidence which is relevant to the Order becomes available after it was made.
