Order

Order: The Registrar is directed to annotate the Register on an interim  basis to show that, for a period of 18 months, you, Mr Wayne Pattinson, when working as a Sonographer (but not when working as a Radiographer) must comply with the following conditions of practice:

1. You must place yourself and remain under the supervision of a workplace supervisor, who must be a Radiographer registered with the HCPC and with a qualification in Sonography.  You must supply details of your supervisor to the HCPC within 2 weeks of the operative date.

2. Every month you must submit for audit by your workplace supervisor, a sample of not less than 5% of your work as a Sonographer.

3. You must with meet your workplace supervisor to discuss the outcome of the monthly audits and comply with such recommendations as the workplace supervisor may make.

4. You must arrange for a report by your workplace supervisor on your clinical performance, to be submitted to the HCPC not less than 14 days before any review of this Order.

5. You must promptly inform the HCPC if you cease to be employed by your current employers (the agencies) or take up any other or further employment.

6. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

7. You must inform the following parties that your registration is subject to these conditions:

A. any organisation or person employing or contracting with you to undertake professional work;

B. any agency you are registered with or apply to be registered with (at the time of application); and

C. any prospective employer (at the time of your application).

Notes

This Order will be reviewed by the Committee no later than 16 April 2019, or earlier if new evidence which is relevant to the Order becomes available after it was made.
