The Registrar is directed to annotate the Register to show that for a period of 18 months from the date that this Order comes into effect, you, Christine Smith-Read, must comply with the following conditions of practice:

1. You must place yourself and remain under the supervision of a workplace supervisor registered with the HCPC and supply details of your supervisor to the HCPC within 7 days of commencing employment. You must attend upon that supervisor every 2 weeks and follow their advice and recommendations.

2. You must work with your supervisor to formulate a Personal Development Plan designed to address the following areas of your practice:-

A. adherence to statutory timescales and those required by your manager in all cases allocated to you;

B. follow the directions of your managers in every respect with regard to the cases allocated to you.

3. Within 3 months of the date of this order you must forward a copy of your Personal Development Plan to the HCPC.

4. You must meet with your supervisor on a 2-weekly basis to consider your progress towards achieving the aims set out in your Personal Development Plan.

5. You must provide information from your supervisor to the HCPC every month towards achieving the aims set out in your Personal Development Plan.

6. You will be responsible for meeting any costs in compliance with any of these conditions.

7. You must promptly inform the HCPC of any new employment or if you cease to be employed.

8. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

9. You must inform the following parties that your registration is subject to these conditions:

• any organisation or person employing or contracting with you to undertake professional work

• any agency you are registered with or apply to be registered with at the time of application

• any prospective employer at the time of your application


This Order will be reviewed by the Committee prior to 11 April 2019, or sooner if new evidence which is relevant to the order becomes available.
