1. In relation to Family A, you:

a. Did not progress the case effectively between 21 July 2015 and 23 December 2015;

b. Did not contact the family and/or did not record that you had contacted the family between 21 July 2015 and 16 May 2016.

 

2. In relation to Service User B, did not organise a replacement bath in a timely manner.

 

3. In relation to Service User C, on 17 March 2017, did not respond appropriately to the concerns raised in that you:

a. Did not identify that they may be at risk and/or make any enquiries into the risk;

b. Did not contact the care agency for up to date information;

c. Did not discuss the case with the duty manager.

 

4. The matters alleged at particulars 1 - 3 constitute misconduct and/or lack of competence.

 

5. By reason of that misconduct and/or lack of competence, your fitness to practise as an Occupational Therapist is impaired.
