Allegation

During the course of your employment as a Social Worker at Hampshire
County Council between 23 August 2016 to 7 November 2016:

1. Between 22 September 2016 and 19 October 2016 in relation to Child A:

a) Did not record sufficient information in the visit notes

b) Did not use the Hampshire County Council visiting template when recording visits.

c) Did not complete and/or record that you had completed visits within the required timescales in accordance with HCC policy.

2. In relation to Family B, you:

a) Did not complete and/or record any visits after 6 September 2016

3. Between 5 September 2016 and 19 October 2016 in relation to Child D:

a) Did not record sufficient information in the visit notes

b) Did not use the Hampshire County Council visiting template

c) Did not complete and/or record that you had completed visits within the required timescales in accordance with HCC policy.

4. In relation to Child E, on or around 21 October 2016, you did not get the Section 20 paperwork signed.

5. In relation to Child F, on 28 October 2016, you:

a) Did not arrive for a visit for which you were the allocated Social Worker

b) Did not return a call to the mother and/or

c) Did not make a record to document that you had returned the call.

6. In relation to Child G, you:

a) did not record sufficient information in accordance with HCC policy in the visit notes for visits conducted on:

i) 23 September 2016

ii) 5 October 2016

b) did not use the Hampshire County Council visiting template when recording visits

c) did not upload visits to ICS within required timescales

7. In respect of Child H, you:

a) did not record sufficient information in accordance with HCC policy in the child protection visit notes for 22 September 2016

b) did not upload visits to ICS within required timescales

8. In respect of Child I, you:

a) did not complete a child protection visit on 19 October 2016 within required timescales in accordance with HCC policy.

b) did not record visits in line with required timescales in accordance with HCC policy.

c) did not record sufficient information and in accordance with HCC policy in the visit notes for:

i) 8 September 2016;

ii) 22 September 2016;

iii) 4 October 2016;

iv) 19 October 2016.

d) did not use the Hampshire County Council visiting template when recording visits

9. In respect of Child J, you:

a) did not record any further visits between 8 September 2016 and 18 November 2016;

b) did not record the visit on 8 September 2016 on ICS within required timescales in accordance with HCC policy;

c) did not record sufficient information in accordance with HCC policy in the visit notes for 8 September 2016;

d) did not use the Hampshire County Council visiting template when recording the visit on 8 September 2016

10. In respect of Child K, you:

a) did not record sufficient information in accordance with HCC policy in the child protection visit notes for:

i) 28 September 2016; and/or

ii) 17 October 2016.

b) did not complete and/or record that you had completed visit within the required timescales in accordance with HCC policy

c) did not use the Hampshire County Council visiting template when recording visits

11. In respect of Child L:

a) did not record sufficient information in the Child Protection visit notes for:

i) 22 September 2016;

ii) 4 October 2016; and/or

iii) 19 October 2016

b) did not record the visit on 4 October 2016 within the required timescale in accordance with HCC policy

c) Did not use the Hampshire County Council visiting template when recording visits

12. In respect of Child M:

a) did not record sufficient information in the child protection visit notes for:

i) 7 September 2016

ii) 22 September 2016

iii) 20 October 2016

b) did not upload visits to ICS within required timescales in accordance with HCC policy

c) did not complete and/or record that you had completed visits within the required timescales in accordance with HCC policy

d) did not use the Hampshire County Council visiting template when recording visits

13. In respect of Child N:

a) did not record sufficient information in accordance with HCC policy in the Child in Need visit notes for:

i) 31 August 2016;

ii) 7 September 2016; and/or

iii) 28 September 2016.

b) did not upload visits to ICS within required timescales in accordance with HCC policy.

c) did not use the Hampshire County Council visiting template when recording visits

14. In respect of Child O:

a) did not record sufficient information in accordance with HCC policy in the Child in Need visit notes for:

i) 2 September 2016 and/or

ii) 23 September 2016.

b) did not use the Hampshire County Council visiting template when recording visits

c) did not upload recordings of visits to ICS within required timescales in accordance with HCC policy

15. In respect of Child P:

a) did not record sufficient information in accordance with HCC policy in the Child in Need visit notes for:

i) 23 September 2016; and/or

ii) 5 October 2016

b) did not use the Hampshire County Council visiting template when recording visits

c) did not upload the record for the visit on 23 September 2016 to ICS within the required timescales in accordance with HCC policy

d) did not record any visits from after 5 October 2016

16. In respect of Child Q:

a) did not record sufficient information in accordance with HCC policy in the Child in Need visit notes for:

i) 6 September 2016;

ii) 27 September 2016;

iii) 4 October 2016; and/or

iv) 28 October 2016.

b) did not use the Hampshire County Council visiting template when recording visits

c) did not upload recordings of visits to ICS within required timescales in accordance with HCC policy

17. In respect of Child R:

a) did not record sufficient information in accordance with HCC policy in the Child in Need visit notes for:

i) 2 September 2016;

ii) 23 September 2016; and/or

iii) 5 October 2016.

b) did not use the Hampshire County Council visiting template when recording visits

c) did not upload recordings of visits to ICS within required timescales in accordance with HCC policy

18. The matters set out in paragraph(s) 1 - 17 constitute misconduct and/or lack of competence.

19. By reason of that misconduct and/or lack of competence your fitness to practise as a Social Worker is impaired.
