Order

Order:  The Registrar is directed to annotate the HCPC Register to show that for a period of 18 months, from the start of this Order, you, Ms Amanda Tyler, must comply with the following conditions of practise:

1. You must restrict your area of practice to adult gynaecology sonography.

2. You must undertake 150 hours of direct supervision and may only return to autonomous practice once signed off as competent by your supervisor.

3. Your Supervision must be Band 7 or above and be an HCPC registered Radiographer or Radiologist working in sonography.

4. You must provide a copy of your Supervisors report to the HCPC confirming your Supervisors view of your competence within 7 days of the Supervisor's decision being received by you I writing.

5. Once autonomous you must provide a report from your line manager every 3 months to the HCPC as to your general standard of your performance with particular regard to the following:

• Ultra sound principles

• Accuracy of reporting

• Case load management

6. You must provide the HCPC with details of your employer, place of employment and details of your supervisor within 7 days of taking up your role.

7. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

8. You must inform the following parties that your registration is subject to these conditions:

a) Any organisation or person employing or contracting with you to undertake professional work;

b) Any agency you are registered with or apply to be registered with (at the time of application); and

c) Any prospective employer (at the time of your application).

Notes

This order will take effect from 24 July 2019.

This order will be reviewed by the Committee no later than 24 January 2020 or earlier if new evidence which is relevant to the Order becomes available after it was made.
