Whilst employed by South London and Maudsley NHS Foundation Trust in the capacity as a Physiotherapist, you:
1. In relation to Patient 1:
a) You did not conduct and/or record one or more of the following on one or more of the dates set out in schedule 1:
I. adequate assessments;
II. adequate physiotherapy intervention;
III. management advice.
b) You did not escalate and/or record escalating clinical issues to the Multi- Disciplinary Team (MDT) on one or more of the occasions set out in schedule 1.
c) You did not follow their seizure care plan on one or more of the following dates:
I. 8 February 2016
II. 25 February 2016
III. 14 March 2016.
d) You did not complete a falls risk assessment and management plan on one or more of the following dates:
I. 11 Feb 2016
II. 9 March 2016
e) On 14 March 2016 you treated Patient 1 alone in his bedroom and did not ensure and/or did not record that another practitioner was present.
2. In relation to Patient 2:
a) you did not conduct and/or record one or more of the following actions on one or more of the dates set out in schedule 2:
I. adequate assessments
II. adequate physiotherapy intervention
III. management advice
b) You did not escalate clinical incidents to the MDT and/or record you had done so on one or more of the following dates:
I. 14 March 2016
II. 1 April 2016
III. 21 April
IV. 25 April
V. 9 May 2016
c) You did not provide clear clinical reasoning or rationale on 9 May 2015 when you massaged the patient’s breast tissue.
d) On 20 or 21 April 2016 you attended Patient 2 but did not write the notes up until 25 April 2016.
3. Did not report Patient 3’s suicide attempts to the MDT when you became aware of them.
4. Did not report when Patient 4 said that “he wish he taken the opportunity yesterday with his few hours of freedom to kill himself” or words to that effect to the MDT.
5. Your actions described at particulars 1 to 4 constitute misconduct and/or lack of competence.
6. By reason of your misconduct and/or lack of competence, your fitness to practise is impaired.
