Whilst registered as a Paramedic, you:

1. On or around 1 January 2017 administered morphine to Patient A without:

a) completing the pre administration checks including blood Pressure

b) supervision, despite being required to be supervised by your employer

c) documenting the reason for administering morphine

2. During a voluntary assessment conducted on 12 June 2018 you did not demonstrate a safe level of practise in that you:

a) In a cardiovascular case study, suggested administering medication and did not:

i) Take a drug, medical and/or social history from the patient

ii) Discuss possible contraindications

iii) Check blood pressure

iv) Use supportive tools and/or drug guidelines to support decision making

b) In a respiratory case study, did not:

i) Discuss possible causes of the presentation other than asthma

ii) Consider environmental factors including possible danger to the patient and paramedic

c) In a nervous system case study, did not:

i) Conduct observations including respiratory rate and pattern

ii) Obtain sufficient information from the patient

iii) Demonstrate clinical reasoning

d) Did not demonstrate safe airway management techniques in that you:

i) Ventilated too quickly

ii) Did not correctly apply a suproglottic airway by inserting the airway back to front

iii) Secured an ET tube with a Thomas Tube Holder which was applied upside down

iv) Did not undertake pre-oxygenation

v) Did not Connect a catheter mount or ATCO2 adapter

vi) Did not correctly identify the purpose of prescribing statins vii) Were unable to explain the mechanics of respiration

viii) Did not identify stroke and / or pulmonary embolism as possible complications related to atrial fibrillation

3.The matters set out in paragraphs 1 – 2 constitute misconduct and / or lack of competence

4. By reason of that misconduct and/or lack of competence, your fitness to practise is impaired.
