Whilst employed as a Paramedic for North West Ambulance Service NHS Trust:

1) On or around 20 December 2016, you attended Patient A and you:

a) Allowed Patient A to be moved from the floor without an appropriate assessment and/or examination having been conducted.

b) Did not carry out an adequate assessment and/or ensure an adequate physical examination was completed.

c) Did not ensure that the Patient Report Form (PRF) was completed adequately in that it did not:

i. Record enough information about the distance of fall.

ii. Record potential loss of consciousness.

iii. Correctly record the level of pain Patient A was experiencing.

iv. Record the lower limb assessments.

v. Record the neurological assessments.

vi. Record any reasons for the advised hospital admission.

vii. Record evidence of the application of the Paramedic Pathfinder.

viii. Include completion of the informed consent and refusal section.

d) Did not perform and/or record performing an Electrocardiogram or ensure that an Electrocardiogram was performed/recorded.

e) Did not offer and/or provide Patient A appropriate pain relief or ensure that pain relief was offered/provided.

f) Did not ensure Patient A was provided with enough information to make an informed decision about whether or not to be conveyed to hospital and/or ensure a record was made of doing so.

g) Did not ensure Patient A was provided with:

i. appropriate referral to other services, and/or record doing so.

ii. adequate safety netting, and/or record doing so.

h) Did not request advice from a senior clinician.

i) Did not check and/or countersign the PRF.

j) Did not discuss the outcome of the Pathfinder with the Patient, and/or record doing so.

k) Did not take and/or record a full patient history or ensure such a history was taken / recorded.

l) Did not undertake a second set of observations or ensure that a second set of observations was taken.

2) The matters described in paragraph 1 constitute misconduct.

3) By reason of that misconduct your fitness to practise is impaired.
