Mr Rauf Ashraf

Whilst registered with the Health and Care Professions Council, and working as a Bank Paramedic, you:

1. On the 15 October 2015 attended incident number 4097119 involving a 90 year old patient at a care home and you;

a) Did not immobilise the patient until she was in the ambulance;

b) Demonstrated inappropriate language and/or unprofessional conduct towards members of staff from the care home;

c) Did not clearly document in in the Patient Care Record (PCR) that the patient had been restrained and / or the reasons for this.

d) Did not administer pain relief to the patient until she was in the ambulance.

e) Did not conduct a mental capacity assessment before restraining the patient.

f) Did not record reasons for administering oxygen to the patient.

2. The matters set out in paragraph 1 constitute misconduct.
