Allegation

During the course of your employment as a registered Social Worker with the National Fostering Agency, you:

1. In relation to Family A and Child B and Child C who were placed with them:
a) did not produce adequate assessment and records.
b) did not make home visits to Child B and Child C and/or record visits between April 2016 and September 2016.
c) did not carry out any assessments in respect of Child B and Child C and/or record any assessments taking place between April 2016 and September 2016.
d) did not complete paperwork to record the carers Annual Foster Care Review in June 2016 for Family A.
e) recorded supervision notes for Child B and Child C dated 15 June 2016 on the carer’s record instead of the children’s records.

2. In relation to Family D:
a) Arranged a Long Term placement of Child E with Family D in November 2013 despite Family D not being approved for Long Term placements.
b) Placed Child F, who was deaf, with Family D for 4 weeks from June to July 2016 and:
i) did not record that Child F absconded on 27 June 2016;
ii) did not arrange and/or record a placement planning meeting;
iii) did not carry out and/or record suitable risk assessments;
iv) did not prepare and/or record safe care plans.
c) did not prepare and/or record bedroom checks for any child placed with the family from June 2015 to June 2016.3. In relation to Person G, approved as a single carer in 2012 and Person H who came to live with him in 2014:
a) did not record the change in circumstances in household records and/or annual reviews.
b) did not carry out and/or record a risk assessment.
c) did not carry out and/or record household checks.
d) did not fully document ‘link visits’ made to provide support, development and advice:
i) in April 2015; and/or
ii) in May 2015; and/or
iii) in June 2015; and/or
iv) in July 2015.

4. In respect of Family I and Child J:
a) between August 2015 to September 2015:
i) did not complete paperwork to evidence bedroom checks recorded as having taken place;
ii) did not carry out and/or record Looked After Child reviews.
b) did not follow up a Schedule 7 Ofsted notification of a serious incident concerning Child J when the police were called to the foster carer’s home.
c) did not make contemporaneous records:
i) between September 2015 and 28 December 2015;
ii) between 28 December 2015 and March 2016.
d) did not produce adequate assessments and recording.

5.The matters set out in paragraphs 1 - 4 constitute misconduct and/or lack of competence.

6. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
