Between March 2012 and December 2014, whilst registered as a SocialWorker, you:

1.Did not carry out the required number of visits and/or did not record visits carried out to the following children:

a) Child 1

b) Child 2

c) Child 3

d) Child 4 and Child 5

e) Child 6

f) Child 7

g) Child 8 and Child 9

h) Child 10 and Child 11

i) Child 12, Child 13, Child 14, Child 15 and Child 16

j) Child 17

k) Child 18

2. Did not complete adequate case recordings in respect of the following children:

a) Child 1

b) Child 2

c) Child 4 and Child 5

d) Child 6

e) Child 7

f) Child 8 and Child 9

g) Child 17

h) Child 18

3. In respect of Child 1:

a) Did not adequately manage the risk to Child 1 whilst in the care of Person A and Person B in that you did not:

i.Carry out an assessment of Child 1 following Child 1’s birth.

ii.Update Child 1’s Child in Need plan.

iii.Carry out an assessment of the parental capacity of Person A and Person B

iv.Follow up the report of a domestic incident between Person A and Person B.

v.Follow up a report that Person A and Person B were considering cohabiting.

vi.Assess the risks posed to Child 1 by Person B.

b) Did not hold a Child in Need meeting before and/or after Child 1’s birth.

c) Did not visit Person A and/or Person B prior to Child 1’s birth.

4. In respect of Child 2:

a) Did not complete a Child in Need plan.

b) Did not convene a Child in Need review meeting after 12 June 2013.

c) Recorded a visit as being a statutory visit when Child 2 was not present.

5. In respect of Child 3:

a) Did not ascertain and/or clearly record Child 3’s wishes.

b) Did not convene a Looked After Child review within the statutory timescale.

6. Did not speak to Child 7 alone and/or record speaking to Child 7 alone.

7. Did not complete a Child in Need plan for Child 10 and Child 11.

8. Did not speak to Child 12, Child 13, Child 14, Child 15 and/or Child 16 alone and/or record speaking to Child 12, Child 13, Child 14, Child 15 and/or Child 16 alone.

9. In respect of Child 17:

a) Did not record an assessment of Child 17’s needs after being placed with her Grandmother.

b) Did not speak to Child 17 alone and/or record speaking to Child 17 alone.

c) Did not convene Child in Need review meetings and/or record Child in Need review meetings as having taken place.

10. The matters described at particulars 1 to 9 constitute misconduct and/or lack of competence.

11. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
