Order

Order: The Interim Conditions of Practice Order made on 24 August 2018 and varied on 22 February 2019, is again varied. The Conditions are:

1. You must not be involved in the direct provision of physiotherapy services to female service users without a chaperone being present throughout the appointment. That chaperone must be a health professional or staff member at your place of work, for example, a receptionist, a physiotherapy support worker, or a healthcare assistant.

2. You must keep a written log of the date, time, patient identifier and name of the chaperone, in respect of each of the occasions when you provide physiotherapy services to female service users. That log must be countersigned by the relevant chaperone in respect of each appointment.

3. You must send a copy of the chaperone log to the HCPC at least 7 days prior to any review hearing.

4. You must inform the HCPC of any disciplinary actions or complaints taken or made against you. Such information shall be provided within 14 days of the event occurring.

5. You must inform the HCPC, within 14 days, if you cease to carry out physiotherapy services for the Well Being Centres, or take up any other or further employment.

6. You must inform the following parties that your registration is subject to these conditions:
(i) any organisation or person employing or contracting with you to undertake professional work;
(ii) any agency you are registered with or apply to be registered with (at the time of application); and
(iii) any prospective employer (at the time of your application).

Notes

This order will be reviewed by the Committee no later than 21 August 2019, or earlier if new evidence which is relevant to the Order becomes available after it was made.
