Allegation
Whilst registered with the Health and Care Professions Council as a Physiotherapist, and during the course of your employment with NHS Grampian, you:
1. In relation to Patient 26, between June 2017 and February 2018, did not:
a) reassess needs having outgrown the buddy roamer;
b) approve use of the GHUG support sling;
c) keep Patient 26’s parents updated;
d) respond to requests for a permanent solution to a sleep system.
2. Did not maintain accurate, timeous records in that, until you left NHS Grampian on 11April 2018, you:
a) made no records in respect of:
i. Patient 14;
ii. Patient 15;
iii. Patient 16;
iv. Patient 21;
v. Patient 24.
b) in relation to Patient 1, made no records beyond September 2017;
c) in relation to Patient 2, made no records after December 2016;
d) in relation to Patient 3, made no records beyond September 2017;
e) in relation to Patient 10, made no records beyond October 2017;
f) in relation to Patient 12 made no records beyond July 2017;
g) in relation to Patient 17, made no record that the patient moved area;
h) in relation to Patient 18, made no records beyond October 2017;
i) in relation to Patient 25, made no discharge report;
j) in relation to Patient 31, made no records beyond May 2017;
k) in relation to Patient 35, made no follow up recorded beyond February 2018;
l) in relation to Patient 38, made no records beyond September 2017;
m) in relation to Patient 39, made no records beyond September 2015;
n) in relation to Patient 44, made no records beyond July 2015;
o) in relation to Patient 46, made no records beyond June 2015;
p) in relation to Patient 47, made no records beyond February 2017;
q) in relation to Patient 49, made no records beyond December 2017;
r) in relation to Patient 50, made no records beyond May 2017;
s) in relation to Patient 60, made no records beyond January 2014;
t) in relation to Patient 61, made no records beyond September 2017.
3. Had in your possession items of equipment:
a) which were meant for service users;
b) which were not provided to service users;
c) which were stored in your home for at least 9 months.
4.The matters set out in paragraphs 1-3 constitute misconduct and/or lack of competence
5. By reason of that misconduct and/or lack of competence, your fitness to practise is
impaired.
