Order

ORDER: The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 18 months, you, Miss Jeannie Hawthorn, must comply with the following conditions:

1. Before you restart work as an Operating Department Practitioner you must inform the following people of these conditions of practice:

A. your supervisor or manager at any place you work; and

B. any agency you are registered with or apply to be registered with (at the time of application).

2. You must not consume any alcohol during the 48 hours before you start any work as an Operating Department Practitioner.

3. You must comply with any alcohol and / or substance misuse policy of any agency or employer when working as an Operating Department Practitioner.

Notes

This Order will be reviewed by the Committee no later than 17 April 2020 or earlier if new evidence which is relevant to the Order becomes available after it was made.
