Whilst registered with the Health and Care Professions Council as a Hearing Aid Dispenser, you:

1. Accepted a cash payment from Patient 2 on or around 21 March 2017 and;

a) repaid your employer, BSHL from your personal bank account, on or around 26 April 2017.

2. Took money from your employer, BSHL, in the form of patient refunds to your personal bank account, in respect of :

a) Patient 40 on or around 23/02/2017 in the sum of £1945.00;
b) Patient 39 on or around 08/03/2017 in the sum of £1095.00;
c) Patient 41 on or around 04/05/2017 in the sum of £50.

3. Received payments from BSHL customers on unrecorded dates and did not give the money to BSHL until a later date in respect of :

a) Patient 3, when on or around 22.04.15 you returned the sum of £495 to BSHL
b) Patient 4, when on or around 15.10.15 you returned the sum of £460.00 to BSHL
c) Patient 5, when on or around 2.02.2016 you returned the sum of £45.00 to BSHL
d) Patient 42, when on or around 31.10.2016 you returned the sum of £445.00 to BSHL
e) Patient 43, when on or around 21.03.2017 you returned the sum
of £820.00 to BSHL
f) Patient 2, when on or around 26.04.2017 you returned the sum of £500.00 to BSHL
g) Patient 1, when on or around 27.04.2017 you returned the sum of £200.00 to BSHL
h) Patient 44, when on or around 16.06.2017 you returned the sum of £14.00 to BSHL
i) Patient 45, when on or around 27.04.2017 you returned the sum of £27.59 to BSHL

4. Amended records in respect of allegation 3(a)-(i) so that the invoice dates and payment dates matched.

5. Recorded fake appointments but went home early on approximately 12 occasions between March – August 2017.

6. Incorrectly amended records to show that Patient 20 had paid the full balance on or around 18.10.2016 rather than writing off the unpaid amount as a bad debt.

7. On or around 01.12.2016 knowingly provided products that were outside of their warranty in respect of Patient 25.

8. Dispensed different hearing aids to the ones ordered and paid for by the patient in respect of :

a) Patient 25 on or around 01.12.2016
b) Patient 26 on or around 04.09.2015
c) Patient 27 In or around October 2015
d) Patient 28 on or around 07.05.2015
e) Patient 29 on or around 22.12.2016

9. Your actions in respect of particulars 1, 2(a)-(c) 3(a)-(i), 4, 5, 6, and/or 8(a)-(e) were dishonest.

10. The matters set out in paragraphs 1-9 constitute misconduct.
11. By reason of that misconduct, your fitness to practise is impaired.
