Order
Order: The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 12 months, you, Ms Frances Lowe, must comply with the following conditions of practice:
1. You must inform the following parties that your registration is subject to conditions:
(a) any organisation or person employing or contracting with you to undertake professional work;
(b) any agency you are registered with or apply to be registered with (at the time of application); and
(c) any prospective employer (at the time of your application).
2. You must confine your professional practice as a Biomedical Scientist to a Band 6, or below.
3. You must place yourself under the supervision of a workplace supervisor who is registered with the HCPC.
4. You must meet with your supervisor on a monthly basis to consider any competence issues that may arise.
5. You must allow your supervisor to provide information to the HCPC about your progress.
6. You should maintain a reflective log detailing incidents which arise where you have made a mistake at work and their potential impact on the patients and colleagues and the steps taken to prevent repetition.
7. You should not hold any line management, or mentoring responsibilities.
Notes
This order will be reviewed by the Committee no later than 26 February 2020, or earlier if new evidence which is relevant to the Order becomes available after it was made.
