Order

The Interim Conditions of Practice Order is varied as below:

The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 18 months, you, Ms Kudzai Makedenge, must comply with the following conditions of practice:


1. You must abstain absolutely from the consumption of alcohol.

2. You must make arrangements for independent testing of your hair for the ingestion of alcohol over a consecutive period of at least 3 months. You must provide the HCPC with the details of the testing arrangements and forward copies of the test results to the HCPC within 7 days of them being received by you and no later than 14 days before the next review hearing.

3. You must comply with any requirements of a treatment plan, including attendance at regular meetings if so required, as recommended by a recognised alcohol support service. You must provide the HCPC with evidence of any attendance at such meetings and/or compliance with any such treatment plan every 4 weeks, until the next review hearing.

4. You must place yourself and remain under the supervision of a workplace supervisor registered by the HCPC, or other appropriate statutory regulator, and supply details of your supervisor to the HCPC within 4 weeks from today’s date. You must attend upon that supervisor as required and follow their advice and recommendations. You must provide the HCPC with evidence of your attendance at meetings with your supervisor every 4 weeks, until the next review hearing.

5. You must inform your GP and any occupational health professional who prepares a report on you about these conditions of practice and request them to provide the HCPC with information about your current health and any treatment you are receiving within 4 weeks from today’s date.

6. You must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment.

7. You must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer.

8. You must inform the following parties that your registration is subject to these conditions:

A. any organisation or person employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with (at the time of application); and
C. any prospective employer (at the time of your application).

9. You will be responsible for meeting any and all costs associated with complying with these conditions.

Notes

This order will be reviewed by the Committee no later than 5 November 2020 or earlier if new evidence which is relevant to the Order becomes available after it was made.
