The Registrar is directed to annotate the Register to show that, for a period of 18 months you, Mr Fazal Karim, must comply with the following conditions of practice:

1. You must not work in nuclear medicine unless directly supervised by a workplace supervisor registered by the HCPC or other appropriate statutory regulator, and you must supply details of your supervisor to the HCPC within 14 days of commencing employment.
 
2. You must meet with your supervisor at least monthly and follow their advice and recommendations. You must send the report from such meetings to the HCPC within 14 days of each meeting.
 
3. You must not undertake preparation of radiopharmaceuticals under any circumstances.
 
4. You must undertake training in radiation protection, covering IRMER 2017 and IRR 2017 regulations, for example the E-Learning for Health (NHS Health Education England) “e-IRMER Module 03 – Legal Requirements” and “e-IRMER Module 06 – Nuclear Medicine”, and provide evidence of execution of this by 31 August 2020.
 
5. You must provide a comprehensive and full CV when applying for any position as a Radiographer.

6. You must promptly inform the HCPC if you take up any other or further employment as a Radiographer.

7. You must notify the following parties that your registration is subject to these conditions:

a. Any organisation or person employing or contracting with you to undertake professional work as a Radiographer;

b. Any agency you are registered with or apply to be registered with as a Radiographer (at the time of application); and

c. Any prospective employer (at the time of your application as a Radiographer).
