Allegation

Whilst registered with the Health and Care Professions Council as a Paramedic and during the course of your employment with London Ambulance Service in the capacity of a Clinical Team Leader, on 15 April 2017:

1) In relation to Patient 1, you:

a) did not conduct your own assessment of Patient 1’s cardiac rhythm on your arrival at scene and/or incorrectly interpreted Patient 1’s cardiac rhythm;

b) accepted the earlier crew's incorrect diagnosis of the patient’s cardiac rhythm, without verification of Pulseless Electrical Activity (PEA), when this was not the case;

c) did not ensure that the defibrillator was placed in such a way that the screen could be seen clearly and/or did not ensure that the screen was checked at appropriate intervals;

d) did not ensure that the defibrillator prompts could be heard and/or were followed;

e) did not correctly record the number of attempts to intubate Patient 1 and/or the size of the ET tube;

f) did not monitor ETCO2 when intubating Patient 1;

g) did not manage the scene and/or did not allocate roles effectively.

2) The matters as set out in paragraphs 1 (a)-(g) constitute misconduct and/or lack of competence

3) By reason of your misconduct and/or lack of competence, your fitness to practise is impaired.
