As a registered Hearing Aid Dispenser (HAD02699) your fitness to practise is impaired by reason of misconduct and/or lack of competence. In that:

1.During your employment with Veritas Hearing between 15 January 2018 and 30 January 2019, you:

a.Met with the company’s customers away from the company’s premises under false pretence and/or during your unauthorised absence from the company;

b.did not adequately record payments charged to patients and/or did not convey payments received from patients to Veritas Hearing, with respect to the patients as set out in Schedule A;

c.Deleted calendar entries from Veritas Hearing’s calendar and/or did not reinstate them;

d.Exported Veritas Hearing customer data on 08 January 2019 without authorisation;
 
e.Did not adequately return and/or account for missing Veritas Hearing items following your resignation from Veritas Hearing on 02 January 2019.

f.Did not adequately carry out diagnostic testing and/or record your clinical reasoning for not doing so and/or update patient notes following consultations, with   respect to patients as set out in Schedule B;

2.Your conduct in relation to 1 (a) to (e) above was dishonest.

3.The matter set out in allegations 1(f) constitutes lack of competence.

4.The matter set out in allegations 1 and 2 above constitute misconduct.

5.By reason of your misconduct and/or lack of competence your fitness to practice is impaired.
