Allegation

Whilst registered with the Health and Care Professions Council as an Occupational Therapist and employed by Greater Manchester Mental Health NHS Foundation Trust you:

1. Did not maintain adequate records, in that you:

a) In relation to Service User A:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not record and/or upload any case notes in relation to Service User A.

b) In relation to Service User B, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not record and/or upload any case notes in relation to Service User B.

c) In relation to Service User C, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not record and/or upload any case notes in relation to Service User C.

d) In relation to Service User D, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not record and/or upload case notes in relation to Service User D.

e) In relation to Service User E, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not take any action on a follow up which was due in February 2017 until 23 July 2017.

f) In relation to Service User F, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) recorded ‘see OT later life support’ but no report was on the system;

iv) did not complete a follow up action with Service User F;

v) did not record any further progress notes.

g) In relation to Service User G, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not complete and/or upload any progress notes.

h) In relation to Service User H, you:

i) On 29 September 2017, visited Service User H and documented the use of

a pellet gun being fired without bullets but did not record any action taken on

the casefile;

ii) did not remove the pellet gun;

iii) did not complete a DATIX form;

iv) did not complete an updated risk assessment;

v) did not inform and/or escalate this incident to a senior manager;

vi) did not inform the police of this incident.

i) In relation to Service User I, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not record any notes following an appointment letter sent on 13 May 2016.

j) In relation to Service User J, you:

i) visited Service User J on 2 October 2017 and did not a record a plan;

ii) did not complete a follow up action with Service User J;

iii) did not complete a further visit after 2 October 2017.

k) In relation to Service User K, you:

i) did not complete a road safety assessment;

ii) did not document your rationale for not completing a road safety

assessment;

iii) did not complete the initial assessment documentation.

l) In relation to Service User L, you:

i) undertook no action and/or follow up on the case after 3 April 2017.

m) In relation to Service User M, you:

i) On 2 October 2017, visited Service User M and recorded ‘see OT doc’ but there was no such documentation on the system.

n) In relation to Service User N, you:

i) On 25 July 2017, recorded ‘visited Service User N. No response at door’,

but did not attempt to re-visit Service User N and/or follow up;

ii) did not complete an action plan;

iii) did not complete an initial assessment in a timely manner.

o) In relation to Service User O, you:

i) did not complete an initial assessment;

ii) did not complete a risk assessment;

iii) did not complete a discharge plan.

2. The matters set out at paragraph 1 amount to misconduct and/or lack of competence.

3. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
