Allegation
As a registered Radiographer (RA49964) your fitness to practise is impaired by reason of misconduct. In that:
1. On 16 and/or 18 July 2018, you did not perform and/or record in a timely manner daily radionuclide calibrator quality assurance checks.
2. On one or more dates between 27 June 2018 and 18 July 2018 you did not perform and/or record in a timely manner daily CT (Phantom) quality assurance checks of the SPECT-CT camera.
3. On 18 July 2018, you retrospectively entered radionuclide calibrator quality assurance records relating to checks scheduled for 16 and/or 18 July 2018:
a. without referring to a record of the results of the check(s);
b. without marking the entries as retrospective.
4. On 18 July 2018 you provided or sought to provide false information to a radiation physicist about radionuclide calibrator records, in that:
a. You stated that the Nuclear Medicine Technologist working in nuclear medicine on 18 July 2018 had performed the radionuclide calibrator quality assurance checks on the day;
b. You had asked or indicated for the Nuclear Medicine Technologist to tell the physicist he had performed the checks, even though he had not.
5. Your conduct in relation to allegations 3 and 4 above was dishonest.
6. The allegations set out in allegations 1 to 5 above constitute misconduct.
7. By reason of your misconduct your fitness to practise is impaired.
