Order
ORDER:The Interim Conditions of Practice Order made on 10 September 2020 is confirmed.
The conditions are:
The Registrant, Ms Sally Bayliss, who is registered as a Physiotherapist with the HCPC, at all material times:
- must not carry out any private practice work
- except in life threatening emergencies, you must not be involved in the direct provision of services without a chaperone being present. The chaperone is to be appointed by your line manager. The name and professional title of the chaperone is to be provided to the HCPC by the Registrant as soon as practicable;
- must maintain a record of:
• every case where you have been involved in the direct provision of services, in each case signed by the chaperone; and
• every case where you have been involved in the direct provision of services to such service users in life threatening emergencies and without a chaperone being present; - must provide a copy of these records to the HCPC 14 days before the date of the next review hearing or, alternatively, confirm that there have been no such cases during that period. You must make these records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC;
- must promptly inform the HCPC if you cease to be employed by your current employer or take up any other or further employment;
- must promptly inform the HCPC of any disciplinary proceedings taken against you by your employer;
- must inform the following parties that your registration is subject to these conditions:
A. any organisation employing or contracting with you to undertake professional work;
B. any agency you are registered with or apply to be registered with (at the time of application); and
C. any prospective employer (at the time of application).
Notes
This order is due to expire on 10 March 2022. Subject to it being extended by the High Court prior to its expiry, it will be reviewed by the Committee no later than or earlier if new evidence which is relevant to the Order becomes available after it was made.
