Allegation
As a registered Radiographer (RA34468) your fitness to practise is impaired by reason of your misconduct. In that:
1. In October 2018, during a hearing before the HCPC’s Conduct and Competence Committee, you presented to the panel quality assurance records which you purported had been signed by
Colleague 1, when this was not the case.
2. The matter set out at paragraph 1 was dishonest.
3. Your actions at paragraphs 1 – 2 amount to misconduct.
4. By reason of your misconduct your fitness to practise is impaired.
