The allegation against you is as follows:
As a registered Radiographer (RA75996) your fitness to practise is
impaired by reason of misconduct and/or a health condition. In that:
- You have a physical and/or mental health condition as set out in
schedule A.
- On 13 December 2019 you informed Colleague 1 that you had discussed
the circumstances set out in allegation 1 above with the HCPC.
- You did not inform the HCPC of the matters set out in allegation 1 above.
- Your conduct in relation to allegation 2 and 3 above was dishonest.
- The matters set out in allegation 2, 3 and 4 above constitute misconduct
- By reason of your misconduct and/or health, your fitness to practise is
impaired.
