Allegation
As a registered Occupational Therapist your fitness to practise is impaired by reason of your misconduct, in that:
1. Between 20 February 2017 and 2 June 2017 you did not keep adequate case records, in that you did not:
a. document specific plans and/or goals of treatment and/or clinical rationale for visiting the Service Users listed in Schedule A;
b. complete discharge reports when ending an episode of care for the Service Users listed in Schedule B;
c. ensure and/or document that follow ups and/or referrals had taken place for the Services Users listed in Schedule C.
2. On 26 May 2017, you did not maintain professional boundaries, in that you:
a. stayed the night at Service User 1’s home address;
b. purchased food and alcohol with Service User 1’s money;
c. provided Service User 1 with alcohol, namely prosecco;
d. did not advise your manager that you were attending Service User 1’s home address outside of working hours.
3. You did not disclose and/or report in a timely manner to senior management that you had stayed overnight at Service User 1’s home address on or around the night of 26 May 2017.
4. You did not document in Service User 1’s records:
a. that you stayed overnight at Service User 1’s home address;
b. the treatment and/or care provided to Service User 1 on 26 and 27 May 2017.
5. On 2 June 2017, you displayed angry and/or aggressive behaviour towards your colleague(s) in that you told a senior member of staff to “shut up” or words to that effect.
