Allegation
Allegations (as amended at this hearing):
1. Between 6 March 2018 and 11 October 2018, you did not maintain accurate and complete records, in that you:
a) Did not complete patient details throughout the documentation for SU1;
b) Did not document a clear rehabilitation management plan for the Service Users in Schedule D;
c) Did not document sufficient information within the records for the Service Users set out in Schedule F.
2. Between 20 February 2018 and 11 October 2018, you did not provide adequate treatment, in that you:
a) Did not complete and/or record an adequate subjective assessment for the Service Users as set out in Schedule A.
b) Did not complete and/or record assessments to the level required of a Band 6 Physiotherapist in relation to the Service Users set out in Schedule E.
3. Between 20 February 2018 and 23 March 2018, you demonstrated a lack of understanding between problems, goals and plans for the Service Users set out in Schedule C.
4. Between 9 May 2016 and 15 February 2019, you did not perform to the independent levels of competency expected of a Band 6 Physiotherapist in that:
a) Your clinical assessment skills of the complex patients was below standard. b) Your clinical reasoning of the complex patients was below standard.
c) Your forward planning and transfer of care decisions, was below standard.
d) You demonstrated an inability to understand research and adoption of evidenced based interventions.
e) You demonstrated an inability to understand clinical guidance and how this relates to clinical practice.
5. You did not inform the HCPC that restrictions had been placed on your duties on 22 October 2018, in that you were removed from the respiratory on call.
6. Your conduct in relation to allegation 5 was dishonest.
7. The matters set out in allegations 1, 2, 3 and 4 above constitute lack of competence.
8. The maters set out in allegations 5 and 6 above constitute misconduct.
9. By reason of your misconduct and/or lack of competence, your fitness to practise is impaired.
