As a registered Radiographer (RA61667) your fitness to practise is impaired by reason of misconduct. In that:
1. Whilst employed at Medical Imaging Partnership you falsified the hourly rate on your bank shift contract from £20 to £25.
2. Your conduct in relation to particulars 1 was dishonest.
3. The matters set out in particulars 1 and 2 constitute misconduct.
4. By reason of your misconduct your fitness to practise is impaired.
