1.      Between 20 July 2019 to 29 September 2019, you worked 11 shifts with a locum agency without permission whilst absent from work at Portsmouth Hospital NHS Trust due to sickness.

2.      Your conduct in relation to particular 1 above was dishonest.

3.      You have a physical and/or mental health condition as set out in Schedule A.

4.      Your actions at particulars 1 and 2 amount to misconduct.

5.   By reason of your misconduct and/or health your fitness to practise is impaired
