Whilst registered with the Health and Care Professions Council as an Occupational Therapist and during the course of your employment with Rhondda Cynon Taf County Borough Council, you:

1. On or around 7 June 2016, recorded, in your calendar, visits to Service Users 1, 2 and 3, but did not record follow up notes on Swift.

2. On or around 9 June 2016, recorded in your calendar that you and visited Service Users 4 and 5 but there were no records for the service users.

3. On or around 20 June, 6, 14 and 15 July 2016, recorded in your calendar that you had visited Service User 6 when you had not done so.

4. On or around 21 June 2016, recorded in your calendar that you had visited Service User 7, when you had not done so.

5. On or around 5 July 2016, recorded in your calendar that the visit to Service User 8 had been cancelled, when the service user could not be identified.

6. On or around 6 and 13 July 2018, recorded a visit in your calendar to see Service User 9 when you had not done so.

7. On or around 12 July 2016, recorded a visit to Service User 10 in your calendar but made no recordings of the visit.

8. The matters described at particulars 1 to 7 were dishonest.

9. The matters described in particulars 1 to 8 amount to misconduct.

10.By reason of your misconduct, your fitness to practise is impaired.
