ORDER: The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 18 months, you, Ian D Clarke, must comply with the following conditions of practice:

1. In relation to any activities for which your HCPC registration is required, you must confine your work to employment by the Northern Health and Social Care Trust.

2. You must not undertake any role related to Cytology, including cervical screening.

3. No less frequently than every three months, you must obtain from the Laboratory Clinical Services Manager at the Northern Health and Social Care Trust, a letter confirming:

a. That you have complied with condition (2) above.

b. That there have been no concerns with the work you have been undertaking.

4. Upon receipt of the letter written by the Laboratory Clinical Services Manager required by condition (3), you must send it to the HCPC.

5. You must inform the HCPC within seven days if you cease to be employed by the Northern Health and Social Care Trust.

6. You must inform the Northern Health and Social Care Trust, and any organisation through which you are undertaking professional training, that your registration is subject to these conditions.

7. You must inform the HCPC within 7 days of becoming aware of:

a. any patient safety incident you are involved in;

b. any investigation started against you: and,

c. any disciplinary proceedings against you.

8. You must allow the HCPC to share with the Northern Health and Social Care Trust details about your performance or compliance with these conditions.

This order will be reviewed by the Committee no later than 13 May 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.
