Allegation

As a registered Occupational Therapist (OT27480) your fitness to practise is impaired by reason of misconduct and/or lack of competence. In that:

1.Between 10 January 2018 and 27 July 2018, you carried out inadequate assessments of one or more of the patients set out in schedule A, in that they were unstructured and/or you did not record sufficient detail with regards to;

a) patient history,

b) presenting difficulties,

c) the impact on their activities of daily living;

d) patient priorities and/or goals

2. You did not keep full, clear and accurate records in that:

a) Your clinical entry on 09 March 2018 in relation to the assessment of Patient 14 is unclear and/or unfocussed and/or lacking in detail;

b) Between 18 January 2018 and 27 July 2018 your ‘occupational circumstances assessment interview rating scale’ (OCAIRS) assessments were not sufficiently detailed and/or complete, in respect of the patients set out in Schedule B.

3.Between 10 January 2018 and 27 July 2018, you did not complete clear treatment plans, for one or more of the patients set out in Schedule C, in that the plan(s):

a) did not indicate the intended outcome(s) and/or overall plan

b) did not identify why intervention(s) had been recommended;

c) included intervention(s) which did not address the reason for referral or patient goals;

d) did not include anticipated timescales

e) were not co-produced with the Patient

4.Between 23 January 2018 and 12 June 2018, you did not complete the treatment plans for the Model of Human Occupation Social Inclusion programme (‘MOHO sips’) in a timely manner, in respect of the patients set out in Schedule D.

5.You did not undertake adequate risk assessments and/or act upon safeguarding concerns, in that;

a) Following an appointment with Service User A on 13 May 2016, you did not inform children’s services, or document your reasoning for not informing them that a 10 year old had;

i. seen a pornographic image/pornographic images;

ii. witnessed violence towards their parents;

b) Following an appointment with Service User A on 13 May 2016, you did not inform adult safeguarding that a vulnerable adult had:

i. been threatened with a knife by another adult; ii. been threatened with a hammer by another adult.

c) Following an appointment with Service User B on 28 April 2016, you did not inform children’s services or adult safeguarding that a vulnerable adult had expressed that they had been thinking of running away with their infant child;

d) On 03 March 2017 during an appointment with Service User C, you did not take appropriate step(s) to intervene after notification that;

i. Service User C had taken a higher than prescribed dose of their mental health medication;

ii. Service User C had intentions to consume a bottle of whisky even though they were on medication for anxiety.

e) between 6 April 2018 and 18 April 2018, you did not take adequate step(s) when you had been unable to make contact with Patient 18;

f) On 05 June 2018, you did not carry out an in-depth risk assessment of risk of suicide in relation to Patient 8;

g) During or following your appointment with Patient 10 on 11 July 2018 you did not formulate an appropriate plan based on the information provided by Patient 10 about her relationship with her son;

 

6. The matters set out in particulars 1 to 5 above constitute lack of competence and/or misconduct.

 

7. By reason of your misconduct and/or lack of competence your fitness to practise is impaired.
