Order
ORDER: The Registrar is directed to annotate the HCPC Register to show that on an interim basis, for the next 18 months, Yohana Habte must comply with the following conditions:
1. You must place yourself and remain under the indirect supervision of a workplace supervisor registered with the HCPC and supply details of your supervisor within 14 days of securing employment as a Radiographer. You must attend upon that supervisor as required and follow their advice and recommendations.
2. You must not work without the direct supervision of an HCPC registered Radiographer when performing any radiographic examination until you have been signed off as competent by your workplace supervisor in that area of radiographic practise.
3. Evidence of the area of radiographic practise in which you have been signed off as competent, must be provided to the HCPC 7 days before any review.
4. You must work with your workplace supervisor to formulate a Personal Development Plan to support your progress as a Radiographer.
5. Within three months of securing employment, you must forward a copy of your Personal Development Plan to the HCPC.
6. You must meet with your workplace supervisor twice a month for the first three months, and thereafter a minimum of once a month (at the discretion of your workplace supervisor), to discuss your progress as a Radiographer. In any new employment, you must start with twice monthly meetings for the first three months.
7. You must provide the HCPC with a report from your workplace supervisor, detailing your progress as a Radiographer 7 days before any review.
8. You must allow your workplace supervisor to provide information to the HCPC about your progress as a Radiographer.
9. You must inform the HCPC within 7 days if you take up any employment or other professional work.
10. You must inform the HCPC within 7 days of becoming aware of:
A. any patient safety incident you are involved in;
B. Any investigation started against you; and
C. Any disciplinary proceedings taken against you.
11. You must inform the following parties that your registration is subject to these conditions:
A. any organisation or person, employing or contracting with you to undertake professional work;
B. Any agency you are registered with, or apply to be registered with to undertake professional work (at the time of application);
C. Any prospective employer for professional work (at the time of your application); and
D. Any organisation through which you are undertaking professional training
12. You must allow the HCPC to share, as necessary, details about your performance, compliance with, and progress under these conditions with:
A. any organisation or person employing or contracting with you to undertake professional work;
B. Any agency you are registered with, or apply to be registered with to undertake professional work (at the time of application);
C. Any prospective employer for professional work (at the time of your application); and
D. Any organisation through which you are undertaking professional training.
13. You will be responsible for meeting any and all costs associated with complying with these conditions.
14. Any conditions requiring you to provide and information to the HCPC is to be met by you sending the information to the offices of the HCPC, marked for the attention of the relevant Case Manager.
Notes
This order will be reviewed by the Committee no later than 30 March 2025 or earlier if new evidence which is relevant to the Order becomes available after it was made.
