Order
The Registrar is directed to annotate the Register on an interim basis to show that, for a period of 12 months, you, Bankole Ogunleye, must comply with the following conditions of practice:
(1) You are only to undertake any activities for which your HCPC registration is required if the organisation that employs or contracts with you can provide you with supervision that will enable conditions (2, 3 & 4) to be complied with by you.
(2) You must place yourself and remain under the supervision of a supervisor who must be registered with the HCPC as a Physiotherapist.
(3) You are to meet with your supervisor at least every two weeks. The supervision must include:
· Your recording of accurate clinical assessments and treatment notes.
· Your recording of informed consent for assessments and treatment.
· Your reflections on your clinical decision-making.
(4) You must obtain a report from your supervisor that includes reference to the specific areas mentioned in condition (3) and provide the same to the HCPC not less than 7 days before any review of this Order.
(5) You must inform the HCPC of the identify of any organisation that employs or contracts with you to undertake activities for which your HCPC registration is required.
(6) You must inform the HCPC of the identity of the supervisor appointed in respect of any activity falling within condition (5).
(7) You must inform the HCPC within seven days of becoming aware of:
(A) any patient safety incident you are involved in;
(B) any investigation started against you;
(C) and any disciplinary proceedings taken against you.
(8) You must inform the following parties that your registration is subject to these conditions
(A) any organisation or person employing or contracting with you to undertake professional work;
(B) any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
(C) any prospective employer for professional work (at the time of your application);
(D) any organisation through which you are undertaking professional training.
(9) You must allow the HCPC to share, as necessary, details about your performance, compliance with, and/or progress under these conditions with:
(A) any organisation or person employing or contracting with you to undertake professional work;
(B) any agency you are registered with or apply to be registered with to undertake professional work (at the time of application);
(C) any prospective employer for professional work (at the time of your application);
(D) any organisation through which you are undertaking professional training.
Notes
This order will be reviewed by the Committee no later than 21 February 2026 or earlier if new evidence which is relevant to the Order becomes available after it was made.
