As a registered physiotherapist (PH106671) your fitness to practice is impaired by reason of misconduct and / or lack of competence. In that:  

1.Between 4 May 2021 and 4 June 2021 you did not screen patients for sinister pathology in that: 

a.You did not carry out red flag screening for Service User A. 

b.You did not carry out red flag screening for Service User B. 

c.You did not screen for Service User C for sinister pathology.

d.You did not carry out red flag screening or ask red flag questions for Service User F.  

e.You did not carry out red flag screening or cranial nerve testing for Service User G.  

f.You did not screen Service User H for sinister pathology.

2.Between 4 May 2021 and 4 June 2021 you did not maintain accurate and complete Service User handovers to colleagues, in that:  

a.You did not document information on Rio and instead provided with information on scraps of paper in relation to Service User B.  

b.You booked Service User C into an appointment with a colleague without 6 providing a handover.  

c.You booked Service User D into an appointment with a colleague with no handover for 3 days.  

3.Between 4 May 2021 and 4 June 2021 you did not manage patient safety, in that you failed to act upon safeguarding concerns. In that:  

a.You did not follow up a safeguarding concern raised regarding Service User B, in that you did not call the registered GP for further information.  

b.You referred Service User D to A&E without arranging a 24-48 hour follow up or arranging an appointment with MSK Triage Service.  

c.You did not liaise with the GP for Service User E after they reported bladder and bowel concerns.  

4.The matters set out in Particulars 1, 2 and 3 above constitute misconduct and / or lack of competence.  

5.By reason of your misconduct and / or lack of competence your fitness to practice is impaired.
