Allegation
As a registered physiotherapist (PH118643):
1. On 20 January 2020, during an assessment with Patient B, you were unable to use the goniometer and/or understand the goniometer readings.
2. On 22 December 2019, you:
a. did not conduct an adequate assessment on Patient C, in that you did not recognise Patient C needed a limb fitting and/or needed to be referred to a rehabilitation class;
b. did not escalate and/or discuss Patient C with a senior practitioner.
3. In relation to Patient D,
a. On 22 September 2020, during a telephone consultation, you did not maintain accurate and complete records, in that you:
i. incorrectly recorded that 'working on the computer' was an aggravation of the injury when that information had not been provided,
ii. incorrectly recorded that the patient's fall was “roughly on 7th July” instead of 2 July 2020,
iii. incorrectly recorded “Ortho to R/V splint”, when the orthopaedics department do not deal with that type of splint,
iv. did not record the following information in relation to Patient D’s pain:
1. specific locations
2. nature
3. intensity
4. behaviour (aggravating and easing factors)
v. did not record the following information:
1. weakness
2. coordination
3. sensation
4. functional disability
b. On 2 October 2020, during a video consultation, you:
i. did not provide adequate treatment in that you used movements to assess the left shoulder which were not clinically appropriate
ii. did not maintain accurate and complete records, in that you:
1. did not adequately record the range of movement for the left elbow and/or left wrist.
2. Did not record adequate clinical observations of Patient D’s injury.
c. On 28 October 2020, during a face-to-face consultation, you:
i. suggested that Patient D should perform an exercise to place their hands against the wall, when Patient D's hand and/or their wrist was not functioning.
ii. did not maintain accurate and complete records, in that you did not record:
1. the range of movement of Patient D’s left elbow in degrees and / or as a proportion of full range
2. Muscle Power
3. the difficulties Patient D was having with their wrist and shoulder
4. details of what had changed for Patient D since your last consultation and/or your observations of their shoulder.
iii. incorrectly recorded 'apparent L wrist drop', when Patient D had been referred for and/or had a wrist drop.
4. In relation to Patient E:
a. On 23 September 2020, during a telephone consultation, you did not maintain accurate and complete records, in that you
i. did not record any information in the section entitled 'Pain A location',
ii. recorded 'pain is there all the time' in the incorrect section of the notes.
iii. in relation to Patient E’s pain, did not record the following information:
a. location
b. nature
c. severity
b. On 26 October 2020, during a telephone consultation, you did not provide adequate treatment in that upon seeing a condition that you did not have sound knowledge of you did not seek advice from a senior member of staff.
5. In relation to Patient F, on 29 September 2020, you:
a. did not provide adequate treatment in that you advised Patient F to remove their prescribed compression stockings without examining Patient F and/or referring them to the practitioner who prescribed the stockings or the issuing pharmacist.
b. did not maintain accurate and complete records in that you did not record a differential diagnosis at the end of the examination.
6. On 1 October 2020, during a telephone consultation, you did not maintain accurate and complete records for Patient G in that you did not record the behaviour and/or pattern of pins and needles in their hand.
7. On 28 October 2020, during a face-to-face examination with Patient H, you did not maintain accurate and complete records, in that you did not record:
a. individual ligament tests
b. specific tests
c. a differential diagnosis
d. a detailed examination
8. On 12 October 2020, during a telephone examination with Patient I, you did not provide adequate treatment in that you did not ask any red flag questions during an initial spinal examination.
9. In relation to Patient J,
a. on 9 November 2020, you did not maintain accurate and complete records in that you inputted information in the wrong sections of the notes.
b. on 12 November 2020, during a video consultation, you:
i. did not ask any red flag questions relating to the lumbar spine
ii. did not maintain accurate and complete records in that you:
1. incorrectly recorded in the notes, ‘flexion – pain’ and/or ‘LSF – hand to knee, pain free’
2. did not include sufficient detail of a detailed examination
iii. did not provide adequate treatment in that you suggested Patient J perform a ‘Chair Squat with Hip External Rotation’ when Patient J was too unstable to undertake the exercise.
10. On 7 December 2020, during a telephone consultation with Patient K, you did not maintain accurate and complete records, in that you:
a. incorrectly recorded that Patient K enjoyed 'PE', when this information had not been provided
b. incorrectly recorded that Patient K could bend their elbow 130 degrees, when you could not observe this from a telephone consultation.
c. did not record that Patient K's mother was present during the telephone consultation.
d. in relation to Patient K’s hand and/or forearm, did not record the following:
1. pain
2. sensation
3. muscle weakness
11. In or around January 2021, during an assessment with Patient L, you were unable to perform a test on Patient L for their anterior cruciate ligament injury.
12. The matters set out in particulars 1 – 11 constitute a lack of competence
13. By reason of the matters set out above your fitness to practise is impaired by reason of lack of competence

Order
Interim Order
The Panel makes an Interim Suspension Order under Article 31(2) of the Health Professions Order 2001, the same being necessary to protect members of the public, being otherwise in the public interest and also in the Registrant’s own interests.
This Order will expire: (if no appeal is made against the Panel’s decision and Order) upon the expiry of the period during which such an appeal could be made; (if an appeal is made against the Panel’s decision and Order) the final determination of that appeal, subject to a maximum period of 18 months.
