As a registered Paramedic (PA02520) your fitness to practise is impaired by reason of misconduct and/or lack of competence. In that:  

1. On 10 December 2019, whilst employed as a Paramedic with the East Midlands Ambulance Service NHS Trust, you did not act in Patient A’s best interests and/or provide adequate care to Patient A, who suffered a cardiac arrest, in that you: 
2. Did not perform an immediate primary survey on Patient A when it was appropriate to do so;
3. Did not deliver a shock to Patient A when advised to do so by the LifePak 15 monitor at approximately 09:38:31 hours and/ or 09:38:45 hours, until 09:39:11 hours;
4. Did not administer Amiodarone Hydrochloride to Patient A when he was in a shockable rhythm after the third shock;
5. Did not immediately and/or adequately assess and/or manage Patient A’s airway;
6. Did not ensure that the Cardiopulmonary Resuscitation (CPR) being undertaken on Patient A by a member of the public was done correctly and/or effectively;
7. Allowed an untrained member of the public to use medical equipment, namely a bag valve mask on Patient A;
8. You were unable to attempt IO (intraosseous) access without clarification on insertion and site suitability from Colleague A;
9. Did not administer and/or record that you had administered a third dose of adrenaline to Patient A between 09:49 and 10:02;
10. i. Did not monitor and/or record Patient A’s Capnography measurement (ETC02) when it was clinically justified and/or appropriate to do so.

2. On or around 14 June 2020, you communicated in a way which was unprofessional, in that you stated to Colleague B, in front of Patient B and/or Patient B’s family, words to the effect of, “the ******** have shafted me again”. 

3. Your actions set out at particulars 1a and/ or 1b and/ or 1d constitute misconduct and/or a lack of competence. 

4. Your actions set out at particulars 1c and/ or 1g and/ or 1h and/ or 1i constitute a lack of competence. 

5. Your actions set out at particulars 1e and/or 1f and/or 2 constitute misconduct. 

6. By reason of your misconduct and/or lack of competence, your fitness to practise is impaired.
