The Registrar is directed to impose the following conditions on the registration of Mr Hardikkumar Nareshbhai Lodaliya for a period of 18 months:

1. You must not be involved in the direct provision of services to female service users without a chaperone being present.

2. If the female service user is under the age of 18, a parent or guardian in loco parentis may act as a chaperone.

3. You must maintain a record of every case where you have been involved in the direct provision of services to female service users, in each case signed by the chaperone and containing their name and role within the Employer’s organisation or their relationship to the Service User (if a parent or guardian in loco parentis who is acting as a chaperone).

4. You must provide a copy of these records to the HCPC on a monthly basis, the first report to be provided within 30 days of the Operative Date (19 December 2025).

5. Alternatively, you must confirm that there have been no such cases during that period on a monthly basis, the first report to be provided within 30 days of the Operative Date (19 December 2025).

6. You must make the above records available for inspection at all reasonable times by any person authorised to act on behalf of the HCPC.

This order will be reviewed by the Committee no later than 18 May 2026 or earlier if new evidence which is relevant to the Order becomes available after it was made.
