Allegation

As a registered Operating Department Practitioner (ODP02414):
1. On 03 August 2023, whilst working at Chelsea and Westminster Hospital,
you:
a) Did not fulfil your role as an Operating Department Practitioner during
your shift without valid reason; and/or
b) Took medication, namely Midazolam, from your workplace, without
authorisation or a prescription; and/or
c) Self-administered medication, namely Midazolam, without
prescription
d) Were intoxicated whilst at work
e) Your conduct set out in Particular 1b and/or 1c was dishonest in that
you took medication that did not belong to you, and which you knew you were
not entitled to.
2. On 03 November 2023, whilst working at Chelsea and Westminster
Hospital, you:
a) Took medication, namely Midazolam, from your workplace, without
authorisation or a prescription; and/or

b) Self administered medication, namely Midazolam without
prescription; and/or
c) Were intoxicated whilst at work; and/or
d) Were in possession of two ampoules of Midazolam without clinical
reason.
e) Your conduct set in particular 2a and/or 2b and/or 2d was dishonest
in that you took medication that did not belong to you, and which you knew
you were not entitled to.
3. On 28 November 2023, whilst working at Croydon Hospital, you:
a) Took medication, namely Midazolam, from your workplace, without
authorisation or a prescription; and/or
b) Self administered medication, namely Midazolam, without
prescription; and/or
c) Were intoxicated whilst at work
d) Your conduct set in particular 3a and/or 3b was dishonest in that you
took medication that did not belong to you, which you knew you were not
entitled to.
4. The matters set in particulars 1, 2 and 3 above constitute misconduct.
5. By reason of your misconduct, your fitness to practise is impaired.
