Allegation

As a registered Paramedic (PA40842):

  1. On one or more occasions between 1 January 2023 and 31 July 2023, as set out in Schedule 1, you took one or more morphine ampoules without clinical justification.

2. Your conduct at particular 1 above was dishonest.

3. On one or more occasions between around 1 January 2023 and 31 July 2023, as set out in Schedule 1, you inaccurately recorded administrations of intravenous morphine in a controlled drug record book.

4. On or about 23 July 2023 you altered Patient C’s clinical record to indicate that you had administered intravenous morphine when you had not done so.

5. On or about 26 July 2023 you altered Patient B’s clinical record to indicate that you had administered intravenous morphine when you had not done so.

6. Your conduct at any or all of allegations 3 to 5 above was dishonest in that in that: a. You knew that the records that you made were inaccurate; and/or, b. You intended to conceal your conduct at particular 1 above.

7. On up to 24 occasions between around October 2022 and April 2023, you failed to keep accurate records of controlled drugs in your possession by not adequately completing your personal drugs book, in that you: a. Left entries incomplete; b. Did not record incident numbers; c. Did not record the year in the date entry; d. Did not ensure that the entries were witnessed and/or accurately record that no witness was available; e. Did not accurately record the running balance entry and/or recorded the ambulance station’s running balance rather than the balance held by you; and/or, f. Did not record where morphine had been signed out from. 

8. Your conduct at any or all of particular 1 to 7 above constitutes misconduct.

9. By reason of the above matters, your fitness to practise is impaired by reason of misconduct.
